As I've mentioned in the past, my son was born at 8 months. This early surprise meant that my hospital bag was only half ready. As a first time mum, I thought I had another month to get my act together. In the end things went ok anyway but it meant that my husband was going to and from the hospital several times a day to go and get/buy missing items.
My advice is to have your bag prepared from 6 months onwards. This will mean you're fully prepared in case your baby decides on arriving a little ahead of time and will also give you time to add items that you might have forgotten.

To those mentioned here I would add a camera (and batteries) for photo's or film. You'd be surprised at how much your baby will change it's appearence in those first few days.
I'm gluten intolerant and therefore also brought gluten free biscuits and snacks. If you have a food intolerence or allergies it's a good idea to have something with you that you can eat in case the hospital isn't prepared to cater for you.
